As a new patient, we ask you to come to the office 10-15 minutes before your appointment to provide identification and verification of insurance.

• There are several versions of these plans. It is the patients responsibility to verify coverage.
• Please consult with your insurance company regarding specific services or visits. We will bill your insurance company as a courtesy, but cannot make any guarantees of coverage or payment from the insurance company.
